Report throughput from a defined eligible population and measure decision delay from the moment a complete packet reaches an authorized owner. Keep active handling, missing evidence, system blocks, and client decisions as separate events. Managers can then improve the part they control without rewarding unsafe shortcuts.

Draw the event timeline before choosing a metric

Take one recent case and mark arrival, eligibility decision, work start, evidence request, evidence receipt, packet completion, owner decision, resumed work, and closure. Name the source for every timestamp. This timeline shows which intervals describe provider handling and which describe dependencies. If the system records only a created date and a closed date, say so. A two-day total cannot reveal whether the worker handled the case for twenty minutes and waited the rest of the time for an authorized answer.

Use the same event names across the queue. Do not let one team call packet complete when evidence is requested while another uses it only after every required source is attached. Definitions should state the triggering action, required fields, timezone, and treatment of reopened items. Keep an effective date. When a definition changes, do not splice the new measure into the old trend without explaining the break.

Define the throughput denominator

Throughput is meaningful only when readers know what could have been completed. State the queue, period, eligible arrivals, opening backlog, exclusions, and completion rule. A hundred received records are not the same as a hundred eligible records if twelve lack source documents and five require decisions outside the role. Count the excluded and waiting items, but do not pretend the worker could complete them through faster data entry.

Avoid selecting only work that closed cleanly. Include returns, corrections, paused cases, and items removed from the queue after admission. Report how the population was selected and whether one item can create several work units. If a case is touched on three days, decide whether throughput counts the case once or counts documented actions. The answer depends on the management question, but it must remain stable enough for comparison.

Start decision delay with a complete packet

A client-decision clock begins when the named owner receives an answerable packet, not when the original request arrives. Define the minimum packet for each decision class. A refund question might require the transaction, customer contact, fulfillment evidence, applicable policy, requested outcome, prior remedy, and exact decision needed. If those elements are missing, the case remains in preparation or evidence collection. This distinction prevents poor packet quality from being mislabeled as slow client response.

The record should preserve packet completion, owner notification, acknowledgement, questions returned, final answer, and expiry. If the owner requests genuinely missing evidence, pause or reclassify the decision clock under the published rule. If the owner asks for information already present because the packet is hard to navigate, record that usability defect separately. The goal is not to make either side look better. It is to show what prevented an answer.

Keep handling and waiting visible together

Build a report with eligible work completed, active handling time where reliably available, items waiting by state, decision packets sent, owner response bands, returned packets, and unresolved consequential cases. Show corrections and reopenings beside the completion count. A high completion number paired with rising corrections is not a clean improvement. A stable completion number with fewer unsupported actions may reflect a safer queue.

Consider one hundred eligible records. Eighty-eight close through routine rules. Twelve contain conflicts and move to client review after the worker assembles evidence. If ten decisions arrive the same day and two remain open, report both the routine throughput and the decision state. Averaging all hundred into one cycle time makes routine execution look slower and makes owner delay harder to see. Splitting the paths gives each manager something they can act on.

Audit incentives created by the report

Ask what a worker or manager might do to improve the number without improving the work. A completion target may encourage premature closure, exclusion of difficult cases, or unrecorded corrections. A response-time target may encourage owners to send quick acknowledgements that do not answer the question. A low escalation target may punish appropriate stops. Review samples from the edges of the definition, including items closed just before a deadline and cases repeatedly moved between states.

Use paired measures and case review to reduce these incentives. Completion belongs with correction and reopening. Decision response belongs with packet completeness and answer quality. Backlog age belongs with state and owner. None of these measures proves business results or individual worth. They are operating signals that help a client locate weak instructions, insufficient review capacity, unreliable sources, or execution that did not follow a settled rule.

Turn the report into a management action

Close each review by naming the problem, evidence, owner, action, and next check. If missing source documents dominate, repair intake. If complete packets wait for one person, add a competent backup or narrow the decision window. If routine handling consumes unexpected time, inspect the steps and systems. If corrections cluster around one rule, improve the instruction and retest. Do not assign every gap to the visible delivery team because that is where the final queue happens to sit.

When late records change a prior period, record the correction rather than silently replacing the number. This gives managers a stable basis for comparing operations while keeping the limits of the data visible.
